Optum HouseCalls seeks a Nurse Practitioner or Physician Assistant to perform annual in-home health assessments for Medicare Advantage members. This non-prescribing, field-based role focuses on preventive care, education, and close collaboration with primary care teams.

The position emphasizes flexible schedules (20–40 hours weekly), no on-call, and strong work-life balance, with opportunities to impact health outcomes in patients' homes.

Qualifications

• ANCC/AANP cert in Family/Adult/Geriatric Primary Care required
• NCCPA cert and active state licensure required for PA role
• Active, unrestricted license or eligibility by start date
• Valid driver’s license and reliable transportation
• Ability to perform in-home assessments across various environments

Responsibilities

• Conduct comprehensive in-home health assessments
• Review medical history and medications
• Perform physical exams, screenings, and point-of-care testing
• Identify and document diagnoses for care management
• Communicate findings to primary care providers
• Educate members on disease management, meds, and adherence
• Address social determinants of health and connect to resources
• Recognize and respond to urgent or emergent situations
