Optum House Calls in Oregon seeks a Nurse Practitioner or Physician Assistant to perform annual in‑home health assessments for Medicare Advantage and other plans. This is a non‑prescribing, field‑based role focusing on preventive care and patient education to close care gaps.

The role collaborates with primary care providers, supports care planning, and offers a flexible schedule with no on‑call, weekends, or holidays.

Qualifications

• NP: ANCC or AANP certification in Family, Adult, Geriatric, or Adult‑Gerontology Primary Care.
• PA: NCCPA certification and active state licensure.
• Active unrestricted license or ability to obtain by start date.
• Valid driver's license and reliable transportation.
• Ability to perform assessments in home environments.

Responsibilities

• Conduct comprehensive in-home health assessments.
• Review medical history and medications.
• Perform physical exams, screenings, and point‑of‑care testing.
• Document diagnoses for care management and planning.
• Communicate findings to primary care providers.
• Educate members on disease management and treatment adherence.
• Address social determinants of health and connect to resources.
• Respond to urgent or emergent situations when needed.
