Job summary

OptumCare is seeking a House Calls NP or PA to conduct in-home health assessments for Medicare Advantage and other health plan members. This non-prescribing, preventive care role focuses on identifying health risks, closing care gaps, educating patients, and collaborating with primary care providers to improve outcomes.

The position offers schedule flexibility with 24-40 hours per week and field-based autonomy, travel to patients’ homes, and meaningful patient interactions in a home setting.

Qualifications

• Active NP or PA certification as applicable (ANCC/AANP for NP, NCCPA for PA).
• Active, unrestricted state license or ability to obtain one.
• Valid driver’s license and reliable transportation.

Responsibilities

• Conduct comprehensive in-home health assessments for Medicare Advantage and other health plan members.
• Review medical history, medications, vitals, and physical examination findings.
• Perform evidence-based screenings and point-of-care testing as appropriate.
