Job summary

Optum HouseCalls is seeking Nurse Practitioners and Physician Assistants to provide annual in-home health assessments for Medicare Advantage and other health plan members. This non-prescribing, field-based role emphasizes preventive care and collaboration with primary care providers.

Flexible schedules, no on-call, and no weekends or holidays. In-home visits across Pierce County, WA, with compensation opportunities including base and bonus structures.

Qualifications

• NP: ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• PA: NCCPA certification and state licensure
• Active, unrestricted license (or ability to obtain by start date)
• Valid driver’s license and reliable transportation
• Ability to perform assessments in a variety of home environments

Responsibilities

• Conduct comprehensive in-home health assessments, including medical history review, medication reconciliation, physical exams, and screenings
• Identify and document diagnoses to support care management and treatment planning
• Communicate findings and recommendations to primary care providers
• Educate members on disease management, medications, and overall health
• Address social determinants of health and connect members with community resources
• Recognize and appropriately respond to urgent or emergent situations

Education
