Job summary

Optum is seeking a House Calls NP or PA to perform in-home health assessments for Medicare Advantage and other health plan members. This field-based role emphasizes preventive care, health risk identification, and care-gap closure with no prescribing responsibilities.

You will review histories, vitals, and medications, provide patient education, and coordinate with primary care providers to improve outcomes. Travel to patient homes is required, with a flexible 24–40 hour schedule.

Qualifications

• Active NP or PA certification (ANCC/AANP or NCCPA)
• Active, unrestricted state license
• Valid driver's license and reliable transportation
• Ability to travel to patients' homes and perform assessments in varied environments

Responsibilities

• Conduct in-home health assessments
• Review medical history, medications, vitals, and exams
• Perform evidence-based screenings and point-of-care testing
• Identify and document diagnoses and care needs
• Communicate assessment findings to primary care providers
• Educate members on disease management, medications, and preventive care
• Address social determinants of health and connect resources
• Recognize urgent health concerns and facilitate appropriate follow-up
