Matrix Medical Network delivers in-home health assessments for at-risk patients across the nation. You will perform comprehensive adult and geriatric evaluations, review health history and medications, and document findings per Matrix guidelines.

As a licensed NP, you will collaborate with physicians, case managers, and caregivers to improve care quality, provide education, and help close care gaps in patients' homes or skilled facilities.

Competenze

• Master’s degree in nursing and active NP licensure in the practicing state.
• Board certification in Adult/Family/GERIATRIC primary care or acute care.
• Current BLS/ACLS or CPR certification required.

Mansioni

• Perform adult/geriatric health assessments at patients' homes or facilities.
• Review medical history, medications, and adherence; document per Matrix guidelines.
• Coordinate with PCPs, social workers, and caregivers for care planning.
