Job summary

Matrix Medical Network provides in-home comprehensive health assessments across the nation. As an NP, you will conduct adult and geriatric assessments, review medical history, medications, and risk factors to improve care quality in patients' homes or SNFs.

You will order tests, work with care teams, educate patients, and ensure thorough documentation following Matrix protocols. Flexible schedule supports weekend and after-hours visits where needed.

Qualifications

• Master’s degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority.
• Board certification in Adult, Gerontology, Family or Emergency NP specialties.
• Current BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads encouraged to apply.

Responsibilities

• Conduct adult/geriatric assessments including history, diagnosis, treatment, education, referrals, and documentation per Matrix guidelines.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers, and caregivers.
• Coordinate with PCP on patient education and follow-up.
• Provide services at the member’s residence (homes or Skilled Nursing Facilities).
