Job summary

Matrix Medical Network is seeking nurse practitioners to perform comprehensive in-home health assessments nationwide. Each visit lasts up to one hour and includes review of medical history, medications, and social factors to identify care gaps and tailor care plans.

Providers administer tests as needed and collaborate with physicians, case managers, and family members. Work locations include patients’ homes or SNFs, with flexible scheduling and potential FT opportunities.

Qualifications

• Master’s degree and NP licensure in state of practice.
• Current NP licensure with prescriptive authority.
• Board certification in one of: Adult, Adult Gerontology, Family, Emergency, or Acute Care.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis, and treatment.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with PCPs, case managers, social workers, and caregivers.
• Provide services at the member’s home or SNF as required.
• Document findings per Matrix guidelines and protocols.
