Job summary

Matrix Medical Network seeks licensed nurse practitioners to perform comprehensive in-home health assessments across the United States. You will review medical history, current health status, medications, and social factors, and document findings to close care gaps.

Assessments are conducted at patients' homes or SNFs, with physician and care-team collaboration. This role offers flexible scheduling, the potential to transition from PRN to full-time, and competitive compensation based on the

Qualifications

• Master’s Degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority or ability to obtain.
• Board certification in Adult, Adult Gerontology Primary Care, Family, Emergency, or Acute Care specialties.
• Current BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads encouraged to apply.
• Valid state driver’s license and auto insurance for residence state.

Responsibilities

• Conduct adult/geriatric health assessments including medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow‑up and documentation according to Matrix guidelines.
• Administer and order point-of-care and lab testing as needed.
• Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
• Collaborate with Primary Care Physician on patient education and follow up as necessary.
• Provide services at the member’s residence which can be in their homes or Skilled Nursing Facilities.
