Job summary

Matrix Medical Network seeks a Nurse Practitioner to perform comprehensive in-home health assessments. Visits last up to one hour and cover medical history, medications, social factors, and gaps in care to improve outcomes.

You will order tests, coordinate with physicians and care teams, and deliver care at members’ homes or SNFs. We offer flexible scheduling with weekend options, ongoing onboarding, and opportunities for licensure support and professional growth.

Qualifications

• Master’s degree in nursing (NP) and licensure to practice as NP in state of residence.
• Board certification in Adult/Family/Gero NP or related specialties is required or pursued.
• Current BLS/ACLS or CPR certification is required.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnoses, treatments, education and referrals.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with PCPs, case managers, social workers and caregivers; document per Matrix guidelines.
• Provide services at members’ residences, including homes or Skilled Nursing Facilities.
• Coordinate follow-ups and patient education to close care gaps.
