Job summary

Matrix Medical Network delivers in-home comprehensive health assessments nationwide, partnering with clinicians to improve care quality and close gaps in chronic conditions. Providers conduct adult/geriatric assessments at members’ residences, including history review, testing, and care coordination.

The role supports flexible scheduling from 8 am–7 pm, Sunday–Saturday, with opportunities for full-time transition and sign-on bonuses.

Qualifications

• Master's Degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority.
• Board certification in Adult, Adult Gerontology, Family or Emergency NP specialties.
• Current BLS/ACLS/CPR certification.
• 1 year NP experience.
• Willingness to travel 80-90% including overnight travel.
• Valid driver's license for state of residence.
• Auto insurance coverage for the state of residence.
• Strong computer skills and familiarity with health/medical record software.
• Excellent verbal and written communication skills.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, referrals, follow-up and documentation.
• Administer and order point of care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers and caregivers as appropriate.
• Coordinate with PCP on patient education and follow up.
• Provide services at member residences, including homes or Skilled Nursing Facilities.
