Job summary

Matrix Medical Network is seeking skilled Nurse Practitioners for in-home assessments across the United States. The role involves comprehensive health evaluations, coordinating with physicians and care team members, and delivering care directly at members' homes or SNFs.

Flexibility is a hallmark—visits occur on a schedule based on your availability, including weekends. The role offers competitive pay based on assessments completed and travel allowances to support nationwide coverage.

Qualifications

• Master’s degree or commensurate NP experience.
• Current NP licensure in state of practice with prescriptive authority.
• AANP/AACN/ANCC board certification in Adult, Gerontology, Family or Emergency NP specialties.
• Current BLS/ACLS/CPR certification.
• 1 year NP experience.
• Able to travel up to 80–90% of the time including overnight travel.
• Valid state driver’s license.
• Auto insurance coverage or ability to obtain.
• Strong computer skills and familiarity with health/medical record software.
• Excellent verbal and written communication skills.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education and referrals.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, and caregivers.
• Provide education and follow-up with PCPs as necessary.
• Deliver services at the member's home or in SNFs.
