Job summary

Matrix Medical Network seeks experienced Nurse Practitioners to perform in-home Comprehensive Health Assessments. Each visit lasts up to an hour, reviewing health, history, medications, and risks with detailed documentation under Matrix guidelines.

Expect travel up to 80-90% across the state and neighboring states. We offer a $1,500 sign-on bonus, flexible scheduling, competitive pay per assessments, mileage reimbursement, and benefits for full-time roles.

Qualifications

• Master's Degree in Nursing and NP licensure in the practice state.
• Current NP licensure and prescriptive authority or ability to obtain.
• Board certification in Adult, Family, or Gerontology specialties.
• Current BLS/ACLS or CPR certification.
• 1 year NP experience preferred.

Responsibilities

• Conduct adult/geriatric assessments including medical history and treatment.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with PCPs, case managers, and other care team members.
• Provide in-home health assessments at members' residences, including SNF visits.
