Matrix Medical Network is seeking a Nurse Practitioner to perform comprehensive health assessments in members' homes and SNFs. Each visit lasts up to an hour and includes medical history review, medication adherence, and risk assessment, with documentation per Matrix guidelines.

Responsibilities include coordinating with PCPs, case managers, and care teams; offering education and follow-up; and conducting point-of-care and lab testing as needed.

Formación

• Master’s degree in Nursing and NP licensure required.
• Current NP licensure in state of practice with prescriptive authority.
• Board certification in appropriate NP specialty desired.
• BLS/ACLS or CPR certification current.

Responsabilidades

• Conduct adult/geriatric assessments including medical history, diagnosis, treatment and follow-up.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers and caregivers as appropriate.
• Provide services at the member’s residence, including homes or SNFs.
• Educate patients and coordinate follow-up with the PCP.
