Job summary
Matrix Medical Network is seeking Nurse Practitioners to perform Comprehensive Health Assessments in patients’ homes or SNFs across the region. Each visit lasts up to one hour, focusing on medical history, medications, and health risks to close care gaps.
You will work with physicians, care teams, and family members to deliver coordinated care, with on-site testing and documentation following Matrix guidelines. A strong nursing background and state licensure are required.
Qualifications
- Master’s degree and NP licensure.
- Current NP licensure with prescriptive authority.
- Board certification in Adult/Adult Gerontology/Family/Acute Care/Psychiatry.
- Valid driver’s license and auto insurance coverage.
- BLS/ACLS certification required.
Responsibilities
- Conduct adult/geriatric assessments including medical history, diagnosis and documentation.
- Administer and order point-of-care and lab testing as needed.
- Collaborate with physicians, case managers, social workers and caregivers.
- Provide services at the member’s residence, including homes or Skilled Nursing Facilities.
- Coordinate patient education and follow-up with the PCP.
