Job summary

Matrix Medical Network seeks experienced Nurse Practitioners to perform in-home comprehensive health assessments, reviewing medical history, medications, and social factors to improve care gaps. Assessments may take up to an hour and are conducted at the member’s residence or SNF.

You will collaborate with PCPs, case managers, and other clinicians, administer testing as needed, and document findings per Matrix protocols.

Qualifications

• Master’s degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice to include prescription authority or ability to obtain prescriptive authority.
• Board certification in Adult, Gerontology, Family, Emergency, or Acute Care specialties.
• Current BLS/ACLS/CPR certification.
• 1 year NP experience preferred; new grads encouraged to apply.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis, treatment, and follow-up per Matrix guidelines.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, and family members.
• Provide education and follow-up with PCPs.
• Provide services at members' residence, including homes or SNFs.
