Job summary

Integrated Services for Behavioral Health (ISBH) seeks a Licensed Counselor or Social Worker to join our home- and community-based team in Licking County, OH. You will assess, plan, and provide therapeutic services under supervision, coordinating care with partners and documenting diligently.

The role requires Ohio licensure as a LSW or LPC, a bachelor's degree in counseling or social work, and a master's degree is preferred. Travel within the service area is expected.

Qualifications

• Excellent communication skills, both oral and written, are required.
• Excellent organizational skills are required.
• Trauma-informed engagement skills are required.
• Ability to use a personal or company vehicle to provide services in homes, schools, and community settings is required.
• Ability to maintain confidentiality of information is required.
• Knowledge of Medicaid billing is required.

Responsibilities

• Conduct biopsychosocial assessments and provisional diagnoses under supervision.
• Engage individuals and families in home and community settings to identify strengths, barriers, and goals.
• Provide therapeutic and supportive interventions within licensure and supervision.
• Develop and update treatment plans using trauma-informed, strengths-based principles.
• Coordinate with internal and external teams to ensure continuity of care.
• Maintain timely, accurate, compliant documentation.
• Deliver care in home and community-based settings.
• Travel within designated service area.
