Qualifications

• Must have licensure as defined by the Ohio Counselor, Social Worker, and Marriage & Family Therapist Board.
• Experience with multi-need individuals and families.
• Broad knowledge of community service systems.
• Willing to participate in and lead cross-systems team-building activities.
• Able to communicate effectively in verbal and written form.
• Must be able to operate in an Internet-based, automated office environment.

Responsibilities

• Assesses and defines strengths and needs of referred individuals and families.
• Directly provides and coordinates clinical and social services.
• Carries out utilization review and quality assurance activities as directed.
• Maintains documentation and participates in program evaluation and planning meetings.
• Meets billing productivity requirements as established by ISBH.
• Other duties as assigned.
