Job summary

Naven Health is a nationwide home infusion nursing network seeking an Infusion Nurse I to provide direct patient care in the home under physician orders. The role involves working under close supervision to ensure patient safety, continuity, and compliance within the infusion program.

Active RN license in the state of practice, 0–2 years infusion nursing experience, and CPR certification are required. Travel to patient homes is expected, with emphasis on documentation, education, and teamwork

Qualifications

• Active and unrestricted RN license in the state of practice.
• Minimum of 0-2 years of infusion nursing experience required.
• Current CPR certification required (an in-person class is required per company policy)

Responsibilities

• Initiates, develops, and implements basic nursing plan of care treatments, evaluating patient progress towards goals.
• Assesses patient needs and physical status at each visit through health data access and patient interview.
• Provides effective and safe teaching using patient-centered care approach for patient and family to achieve independence with prescribed therapy and care needs.
• Communicates with Clinical team, medical providers, patients, and families/caregivers to facilitate continuity of care and documents in real time.
• Coordinates discharge planning and prepares discharge summaries with patient instructions and progress reports.
• Demonstrates compliance with agency operations, policies and procedures, and applicable regulations.
• Maximizes work efficiency through use of computers to validate plan of treatment orders and communicate care provided.
• Responds promptly to patient requests and initiates emergency procedures as necessary.
• Observes HIPAA and confidentiality requirements and upholds privacy standards.
• Attends and completes required training modules and may mentor other nurses.
