Job summary

Naven Health is seeking an Infusion Nurse I to provide direct patient care in home or alternative infusion sites under a physician’s plan of care. You will work under close supervision while ensuring patient safety and continuity of care.

This role requires an active RN license, 0–2 years infusion nursing experience, and CPR certification. Travel 100% is required to serve patients across our network, with comprehensive benefits and ongoing training.

Qualifications

• Active RN license in the state of practice.
• Minimum of 0-2 years of infusion nursing experience.
• Current CPR certification required (in-person class per company policy).

Responsibilities

• Initiates, develops, and implements basic nursing plan of care treatments, evaluating patient progress towards goals.
• Assesses patient needs and physical status at each visit through health data access and patient interview.
• Provides effective and safe teaching using patient-centered care for patient and family to achieve independence with prescribed therapy.
• Communicates with the Clinical team, medical providers, and families to facilitate continuity of care and documents in real-time.
• Coordinates discharge planning and prepares discharge summaries with patient instructions and reports progress and goals.
