Job summary

Anchor Health in California is seeking an experienced Hospice Nurse to plan, organize and direct home-based hospice care. You will implement the patient plan of care, coordinate with the Interdisciplinary team and family, and communicate patient needs to physicians to support seamless care.

Requirements include NLN-accredited nursing program graduate, CA RN license and CPR certification; a minimum of one year in professional nursing, with two years in public health, home care or hospice

Qualifications

• Graduate of NLN-accredited nursing program.
• Current CA RN license and CPR certification.
• RN with at least 1 year of experience in professional nursing (last 3 years).
• 2+ years in public health, home care or hospice preferred.
• Must supervise hospice aides when needed.
• Excellent observation, communication and problem-solving skills.
• Must be able to lift and transfer patients and have a valid driver's license.

Responsibilities

• Provide professional nursing care using all elements of the nursing process.
• Assess and evaluate patient status and revise the care plan as needed.
• Communicate patient needs to physicians and the Interdisciplinary team.
• Develop a patient- and family-centered care plan with input from all parties.
• Complete and maintain accurate clinical notes and documentation.
• Coordinate care with community health professionals and physicians.
