Job summary

Pallatus Health is seeking a compassionate Hospice Registered Nurse (RN) to provide expert nursing care, support families, and ensure comfort and dignity for patients in their final stages of life. You will coordinate care for hospice patients in homes, facilities, and community settings, working with physicians, social workers and chaplains to honor patient wishes.

The role requires strong clinical skills, 2+ years of nursing experience, and a CA RN license with CPR certification.

Qualifications

• Current RN license in the state of practice
• Minimum 2 years of clinical nursing experience
• Strong assessment and critical thinking skills
• Excellent communication and interpersonal abilities
• Ability to work independently and make sound clinical judgments
• Comfortable discussing end-of-life issues with patients and families
• Valid driver’s license and reliable transportation for home visits
• CPR certification required

Responsibilities

• Provide skilled nursing assessments, care planning, and direct patient care for hospice patients
• Manage pain and symptom control protocols under physician supervision
• Educate patients and families on disease progression, comfort measures, and what to expect
• Coordinate care with interdisciplinary team members including physicians, social workers, and chaplains
• Respond to urgent calls and provide crisis intervention when needed
• Document patient care activities and maintain accurate medical records
• Support families through the dying process and provide bereavement guidance
• Ensure compliance with hospice regulations and quality standards
• Provide mentorship and guidance to nursing assistants and volunteers
