Qualifications

• EMR experience with Homecare Homebase/Point Care is a plus.
• Patient-centered approach and empathy required.
• Ability to collaborate with physicians and interdisciplinary teams.

Responsibilities

• Assess and evaluate patient status in home hospice care.
• Develop and revise individualized care plans.
• Provide palliative nursing care and family education.
• Act as Case Manager when assigned to a patient/care team.
• Maintain accurate clinical documentation and reporting.
• Communicate effectively with physicians and community partners.
