Qualifications

• Graduate of an accredited nursing program.
• Current California RN license.
• Current CPR certification.
• Experience in public, home care, or hospice nursing preferred.

Responsibilities

• Complete initial and ongoing comprehensive patient and family assessments to determine hospice needs.
• Perform comprehensive physical assessments and document medical history.
• Conduct home visits and evaluate patient status.
• Initiate and revise the plan of care as patient needs change.
• Develop care plans with goals incorporating nursing diagnoses and family involvement.
• Administer medications and treatments as prescribed.
• Participate in Interdisciplinary Group Meetings.
• Maintain accurate clinical notes on patient condition and care provided; supervise ancillary staff when required.
