Job summary

Affinity Hospice Management LLC seeks a Registered Nurse to plan, organize and direct hospice care in patients' homes and communities. The RN will assess, develop, implement and revise plans of care with emphasis on family involvement and education.

The role requires nursing expertise, community health experience, and strong collaboration with the interdisciplinary team to ensure high-quality palliative care and patient comfort in home settings.

Qualifications

• Graduate of NLN-accredited nursing program
• Current RN licensure in the state
• At least 1 year of professional nursing in last 3 years, or BSN with RN license
• Experience preferred in public health, home health, or hospice nursing

Responsibilities

• Complete comprehensive patient and family assessments and plan of care
• Provide professional nursing care using the nursing process
• Regularly re-evaluate patient/family needs and revise the plan of care
• Administer medications and treatments as prescribed
• Counsel patients and families on nursing and related needs
• Act as Case Manager when assigned and coordinate care
• Participate in interdisciplinary team meetings
