Chapters Health System is seeking an RN Case Manager to assess patient and family needs, coordinate the Plan of Care with the IDT, and provide clinical, palliative and supportive care to keep patients in their home environment when possible.

Requirements include an active RN license in Florida, 1+ year nursing experience (hospice/hospital preferred), Alzheimer’s training for PACE locations, EMR/EHR experience, and a valid driver’s license with reliable transportation.

Qualificações

• Must hold RN license in the state you will work.
• Minimum 1 year nursing experience; hospice or hospital preferred.
• PACE location requires Alzheimer’s Disease and Related Dementias Training.
• Experienced with EMR/EHR systems.
• Mobile driver with valid license and auto insurance per company policy.
• Reliable transportation to meet visit schedule.
• Ability to communicate effectively in English; verbal and written.
• Ability to visit participants at home and perform essential functions (lifting, bending, etc.).

Responsabilidades

• Provide direct care to patients and families as part of the IDT, including psychosocial and physical needs.
• Coordinate individualized Plan of Care reflecting evolving patient needs.
• Educate patients and families about disease, care planning, palliative interventions, and safety.
• Conduct home visits to assess safety, medication and nutritional needs; report changes to IDT.
• Participate in IDT meetings; document communications and follow SBAR process.
