Job summary

Gentiva Hospice seeks an experienced RN Case Manager to lead in-home patient care, collaborating with physicians and the interdisciplinary team to deliver comprehensive, compassionate hospice services. You will assess, plan, and implement individualized care plans while educating patients and families in end-of-life care and symptom management.

The role requires strong clinical judgment, excellent communication, and the ability to coordinate care across settings to ensure seamless transitions

Qualifications

• A minimum of one (1) year experience as an RN, either in acute care hospital, oncology, long‑term care, hospice or home health.
• Hospice or palliative care experience preferred.

Responsibilities

• Perform comprehensive patient assessments and reassessments including in-person/telephonic RN assessments/screenings and provide education to patients, family members and caregivers as appropriate.
• Deliver skilled nursing interventions and symptom management aligned with goals of care.
• Develop and update individualized plans of care including patient visit frequency to meet individual needs and risk identification.
• Coordinate care across interdisciplinary teams including consultation with physicians and nurse practitioners.
