Job summary

Gentiva is seeking a dedicated Registered Nurse Case Manager for full-time, dayshift coverage. You will lead in-home hospice care, conduct comprehensive assessments, and coordinate with an interdisciplinary team to implement personalized plans of care.

The role emphasizes patient/family education, symptom management, and timely documentation, within a comprehensive Total Rewards package.

Qualifications

• Active RN license in the state of employment.
• Minimum 1 year RN experience in hospice, palliative care, or home health.
• CPR certification required.

Responsibilities

• Be the primary point of contact for patients and families throughout the hospice journey.
• Deliver and document skilled nursing care per individualized Plan of Care in home settings.
• Perform initial and ongoing assessments and adjust care as needed.
• Collaborate with IDG to develop and refine care plans.
• Provide emotional, physical, and spiritual support to patients and loved ones.
