Job summary

Signify Health, part of CVS Health, is seeking a Per Diem Physician to perform in-home health evaluations, engaging with patients in their own homes and helping bridge gaps in care. The role includes about 12 hours weekly, travel to local and licensed states, virtual visits when needed, and performing health evaluations with medication and history reviews, physical exams, and diagnostics as ordered.

You will be supported by malpractice coverage and supplied with the tools to deliver

Qualifications

• Active, unrestricted medical license in the applicable state.
• MD or DO degree from an accredited medical school.
• Multi-state licenses preferred.

Responsibilities

• Join a national network of clinicians delivering in-home health evaluations.
• Travel locally to licensed states as needed.
• Conduct in-home visits and virtual visits as required.
• Provide diagnostic and preventive services as indicated.
