Job summary

CVS Health / Signify Health is seeking a Per Diem Physician to provide in-home health evaluations. Clinicians will engage with members in their homes, bridge gaps in care, and perform assessments without prescribing medications or ordering labs.

This is a part-time, per diem role with travel within licensed states and occasional virtual visits. The position emphasizes compassionate care, adherence to health plan requirements, and the use of an iPad-based clinical workflow to complete

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses (preferred).

Responsibilities

• Provide in-home health evaluations.
• Engage with people in their homes and bridge gaps in care.
• Conduct medication and medical history reviews.
• Perform physical evaluations; order tests as needed per health plan.
• Travel locally within licensed states; may include some travel.
• Utilize iPad and clinical workflow for each evaluation; no prescribing or lab orders.
• Conduct virtual visits as needed.
