Signify Health, part of CVS Health, is seeking a Per Diem Physician to provide in-home health evaluations. Engage with people in the comfort of their homes and bridge gaps in care, joining a national clinician network.

The role offers part-time hours (approximately 12 per week), travel within licensed states, and virtual visits as needed, with a base hourly pay range and comprehensive benefits for eligible hours.

Qualifications

• Active, unrestricted licenses in coverage area(s).
• Multi-state licenses preferred.

Responsibilities

• Conduct in-home health evaluations for CVS Health members.
• Travel locally and within licensed states as needed.
• Conduct virtual visits as needed.
• Provide Diagnostic & Preventive Services as indicated.
• Use iPad and clinical workflow; no prescribing or lab orders.
