Qualifications

• Active, unrestricted medical license(s) in the coverage area.
• Multi-state licenses are preferred.

Responsibilities

• Provide in-home health evaluations during 45‑minute visits.
• Review medications and medical history, perform physical evaluation.
• Conduct Diagnostic & Preventive Services as ordered by health plan.
• Travel locally and within licensed states, as needed.
• May conduct virtual visits as needed.
