Job summary

Signify Health, part of CVS Health, seeks a Per Diem Physician to provide in-home health evaluations. You will engage with patients in their homes, review medical history, perform physical assessments, and support care coordination without prescribing or ordering tests unless needed by health plans.

The role involves traveling locally to licensed states, conducting virtual visits as needed, and joining a national network of clinicians dedicated to improving health outcomes.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses (preferred).

• Conduct virtual visits as needed.
• Provide all Signify Health services as indicated including Diagnostic & Preventive Services products.
