Job summary

Matrix Medical Network is seeking experienced NP/PA clinicians to perform comprehensive in-home health assessments, reviewing health history, medications, and living environments to identify gaps in care. You will work with a team of physicians, case managers, and caregivers to coordinate next steps and ensure high-quality patient outcomes.

Role requires active NP/PA licensure, board certification, and willingness to travel up to 80–90% across states.

Qualifications

• Current NP/PA licensure in state of practice with prescriptive authority or ability to obtain
• NCCPA, AANP, AACN or ANCC board certification in relevant specialties
• Experience as NP or PA
• Ability to travel extensively (80–90%) across states as needed
• Proficient with computer systems and medical records software

Responsibilities

• Conduct adult/geriatric health assessments including medical history, diagnoses, treatments, health education, referrals, follow-up, and documentation per Matrix guidelines
• Administer and order point-of-care and lab-based testing as needed
• Collaborate with physicians, case managers, social workers, and caregivers
• Coordinate with PCP for patient education and follow-up
• Provide services at members’ residences (homes or Skilled Nursing Facilities)
