Job summary

VNA Health Group, through Visiting Physician Services (VPS), delivers high-quality in-home primary care to older adults who are homebound or medically complex. Our interdisciplinary team also provides telehealth, diagnostics and wound care to support safe, comfortable care in the patient’s home.

As a Physician Assistant in VPS, you will assess and manage care plans, collaborate with a skilled team, and participate in QAPI initiatives to maintain excellence in home-based care.

Qualifications

• Completion of Physician Assistant accredited program.
• Licensure as a Physician Assistant (PA) in NJ with prescriptive authority; maintain a CDS and DEA.
• National certification as a PA with AAPA.
• Experience providing care to Adult or Geriatric patients.
• 2+ years of Physician Assistant experience and/or significant experience as a registered nurse.
• Excellent communication, organizational and interpersonal skills.
• Strong computer skills.

Responsibilities

• Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers.
• Assesses, diagnoses and develops appropriate plan of care to treat homebound patients.
• Monitors the patient’s condition and responsiveness to the treatment provided.
• Acts as an agency representative with patients and their families as well as with other health agencies, groups and organizations.
• Participates in organizational wide QAPI program and attends agency sponsored in-service programs.
• Adheres to agency infection control and safety policies.
• Adheres to the organization’s policy in regards to absenteeism and appearance.
