Job summary

VNA Health Group, via Visiting Physician Services (VPS), delivers comprehensive in-home medical care for older adults who are homebound or medically complex. The Physician Assistant will provide high-quality care, perform assessments, manage chronic conditions, and collaborate with a multidisciplinary team to keep patients safe and comfortable at home.

This role emphasizes adherence to infection control, participation in QAPI programs, and compassionate, patient-centered care in New Jersey.

Qualifications

• Completion of Physician Assistant (PA) accredited program.
• Licensure as a Physician Assistant (PA) in NJ with prescriptive authority; maintain a CDS and DEA.
• National certification as a PA with AAPA.
• Experience providing care to Adult or Geriatric patients.
• 2+ years of Physician Assistant experience and/or significant experience as a registered nurse.
• Excellent communication, organizational and interpersonal skills.
• Strong computer skills.

Responsibilities

• Assesses, diagnoses and develops appropriate plan of care to treat homebound patients.
• Monitors the patient's condition and responsiveness to the treatment provided.
• Acts as an agency representative with patients and their families as well as with other health agencies, groups and organizations.
• Participates in organizational wide QAPI program and attends agency sponsored in-service programs.
• Adheres to agency infection control and safety policies.
• Adheres to the organization's policy in regards to absenteeism and appearance.
