Job summary

VNA Health Group's Visiting Physician Services in New Jersey seeks a Nurse Practitioner to deliver high-quality in-home primary care to homebound patients. You will assess, diagnose, and plan care to improve quality of life while working within an interdisciplinary team.

The role requires NJ APN licensure with prescriptive authority, national NP certification (Adult or Gerontologic), and strong communication and computer skills.

Qualifications

• Master’s degree in Nursing from an NLN-accredited program.
• NJ Advanced Practice Nurse licensure with prescriptive authority; CDS and DEA.
• National certification as Adult or Geriatric NP.
• 2+ years APN experience or significant RN experience.
• Excellent communication, organizational and interpersonal skills.
• Strong computer skills.

Responsibilities

• Supports organization’s mission with excellence and positive relationships.
• Assesses, diagnoses and develops care plans for homebound patients.
• Monitors patient condition and response to treatment.
• Represents the agency with patients, families and external health bodies.
• Participates in organizational QAPI programs and in-service trainings.
• Adheres to infection control and safety policies.
• Adheres to attendance and appearance policies.
