Qualifications

• Licensed as a physician in New Jersey with education and experience in primary care settings.
• Geriatric experience recommended.
• Board certified in family practice or internal medicine.
• Active CDS and DEA license.
• Excellent verbal and written communication skills.
• Excellent organizational and interpersonal skills.
• Car and valid NJ driver’s license.

Responsibilities

• Provide direct medical care and consultation to homebound primary care patients within established productivity guidelines.
• Monitor patient conditions and adjust treatment plans based on clinical needs and responsiveness.
• Serve as a compassionate, professional representative of the agency to patients, families, and community partners.
• Collaborate with colleagues across the interdisciplinary team.
• Contribute to both formal and informal staff education.
• Participate in community outreach efforts to promote Visiting P services.
• Maintain accurate, timely documentation in the Electronic Medical Record.
• Ensure compliance with NJ Department of Health regulations and all relevant licensing and accrediting requirements.
• Adhere to all infection control and safety policies.
• Uphold the organization’s mission through excellence, professionalism, and positive teamwork.
