Job summary

WellBe Senior Medical is seeking a licensed Physician Assistant to deliver in-home primary and acute care to frail older adults in West Virginia. You will join a physician-led team providing concierge-style care across the care continuum, from chronic disease management to end-of-life support.

You will conduct in-home assessments, order tests, manage medications, and coordinate with an interdisciplinary team to keep patients safe at home. Travel to patient homes is required within the market.

Qualifications

• Master’s degree in Physician Assistant Studies from an ARC-PA accredited program and WV license.
• 3-5 years physician assistant experience (no new grads).
• 2-3 years prior healthcare experience in related roles.
• Active WV PA license and NCCPA certification.
• BLS, NPI, CPR, DEA licenses (active).

Responsibilities

• Provide in-home visits and longitudinal primary care.
• Collaborate with clinicians in care community.
• Screen, assess, and document patient plans.
• Order and interpret diagnostic tests; follow up.
• Prescribe and manage medications per state policy.
• Maintain accurate patient records and chart reviews.
• Support palliative and end-of-life care as needed.
