Qualifications

• 2+ years of APN experience and/or significant experience as a registered nurse.
• Experience in in-home medical care is preferred.
• Ability to adhere to agency infection control and safety policies.

Responsibilities

• Responsible for delivering high quality care to homebound patients.
• Assess and develop appropriate plans of care.
• Monitor patients' condition and treatment responsiveness.
• Act as a representative for patients and families.
• Participate in organization-wide QAPI program.
