Job summary

Matrix Medical Network seeks a skilled Nurse Practitioner to perform comprehensive in-home health assessments. You will review health history, medications, and risks, documenting findings for care plans with PCPs and care teams.

Assessments occur at members' homes or SNFs, with travel as needed. Qualified candidates hold NP licensure in their state, prescriptive authority, and board certification in relevant specialties.

Qualifications

• Master’s Degree or commensurate NP experience.
• Current NP licensure in state of practice with prescriptive authority.
• Board certification in Adult, Gerontology, Family or Emergency care.
• Current BLS/ACLS or CPR certification.
• 1 year NP experience preferred; new grads welcome.
• Valid driver’s license and auto insurance in residence state.

Responsibilities

• Conduct adult/geriatric health assessments in patient homes or SNFs.
• Review medical history, medications, and care gaps with documentation.
• Coordinate with PCPs, case workers, and family members for follow-up.
• Order point-of-care and lab testing as needed.
