Job summary

Matrix Medical Network is seeking a Nurse Practitioner to perform comprehensive in-home health assessments, reviewing medical history, medications, and social factors to close care gaps.

Providers conduct geriatric assessments, order tests, and collaborate with PCPs, case managers, and family members, delivering care at the member's residence, including homes or SNFs, with flexible scheduling and opportunities for full-time roles.

Qualifications

• Master’s degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority if needed.
• Board certification in Adult, Gerontology, Family or related specialty.

Responsibilities

• Conduct adult/geriatric assessments including history, diagnosis and treatment.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers and family members.
• Coordinate with PCP for patient education and follow-up.
• Provide services at the member’s residence (home or SNF).
