Job summary
Matrix Medical Network seeks a qualified Nurse Practitioner to perform comprehensive in-home health assessments for older adults and at-risk individuals. You will review medical history, medications, and social factors during visits conducted at patients’ homes or SNFs, delivering timely documentation and appropriate referrals.
Ideal candidates hold NP licensure with prescriptive authority and board certification.
Qualifications
- Master’s degree in Nursing and NP licensure.
- Current NP licensure in state of practice.
- Board certification in Adult/Geriatric/Family NP preferred.
- BLS/ACLS or CPR certification.
- Minimum 1 year NP experience preferred.
Responsibilities
- Perform adult/geriatric assessments including medical history, diagnosis, treatment, education, referrals, and documentation per Matrix guidelines.
- Administer and order point-of-care and lab-based testing as needed.
- Collaborate with physicians, case managers, social workers, family members, and other care team.
- Coordinate patient education and follow-up with Primary Care Physician (PCP).
- Provide services at members’ residences, including homes or Skilled Nursing Facilities.
