Job summary

Matrix Medical Network is seeking Nurse Practitioners to conduct Comprehensive Health Assessments at members' homes or Skilled Nursing Facilities. Each visit is designed to capture medical history, health status, and medication adherence to identify care gaps and improve outcomes.

Qualifications

• Must hold NP licensure in state of practice.
• AANP, AACN, or ANCC board certification required.
• 1 year NP experience required.
• Ability to travel up to 80-90% of the time across the assigned state and neighboring states.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnoses and treatment, health education, referrals, and documentation per Matrix guidelines.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, family members, and other care team members.
• Coordinate with the PCP on patient education and follow-up.
• Provide services at the member's residence, including homes or Skilled Nursing Facilities.
