Job summary

Matrix Medical Network is hiring Nurse Practitioners to perform comprehensive in-home health assessments across the assigned region. You will review medical history, medications, and social determinants to identify care gaps and coordinate follow-up with PCPs and care teams.

Providers work in members’ homes or skilled nursing facilities, with travel up to 80–90% and flexible scheduling. Competitive pay, mileage reimbursements, and support for licensure are offered.

Qualifications

• NP licensure in state and prescriptive authority as needed.
• 1 year NP experience.
• Board certification in Adult/Family/ Gerontology specialties.
• BLS/ACLS certification.

Responsibilities

• Conduct adult/geriatric assessments including medical history and diagnoses.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers and caregivers.
• Provide patient education and follow-up with PCP as necessary.
