Job summary

Matrix Medical Network seeks Nurse Practitioners to perform comprehensive in-home health assessments for at-risk populations. Each visit lasts up to one hour and includes medical history review, diagnoses, treatment planning, and follow-up coordination with PCPs.

Providers administer tests as needed and coordinate with case managers, social workers, and family members to close care gaps. Employment offers flexible scheduling, mileage reimbursement, and options for licensure support.

Qualifications

• Master’s Degree in Nursing or equivalent
• NP licensure in state of practice with prescriptive authority
• AANP/AACN/ANCC board certification relevant to adult-gerontology/family care
• Current BLS or ACLS certification
• 1 year NP experience preferred, new grads welcome
• Valid state driver's license and auto insurance coverage
• Strong computer skills and EHR familiarity

Responsibilities

• Review member health history and medications during visits
• Conduct adult/geriatric assessments with documentation per Matrix guidelines
• Order and administer point-of-care and lab tests as needed
• Collaborate with physicians, case managers, social workers and caregivers
• Provide services at the member’s home or Skilled Nursing Facility
