Matrix Medical Network seeks a Nurse Practitioner to perform comprehensive in-home health assessments, reviewing medical history, medications, and social determinants with patients in their homes or Skilled Nursing Facilities. You’ll collaborate with PCPs, case managers, and families to close care gaps and improve outcomes.

The role requires NP licensure in the state of practice, board certification in an appropriate specialty, BLS/ACLS/CPR, and 1 year of NP experience preferred.

Qualificações

• Master’s Degree in Nursing or commensurate NP experience.
• Current NP licensure in state of practice with prescriptive authority.
• AANP/AACN/ANCC board certification in Adult/GERO/Family/ED/Acute Care.
• Current BLS/ACLS/CPR certification.
• 1 year NP experience preferred; new grads welcome to apply.
• Valid state driver’s license; auto insurance coverage as required.
• Strong computer skills; familiarity with health records software.

Responsabilidades

• Conduct adult/geriatric assessments including medical history, diagnosis, treatment and referrals.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers, and family members.
• Coordinate patient education and follow-up with PCP as needed.
• Provide services at the member’s residence, including homes or SNFs.
