Matrix Medical Network is seeking a qualified Nurse Practitioner to conduct comprehensive home health assessments for adults and geriatric patients. This role involves reviewing medical history, medication adherence, and social determinants to identify care gaps and improve outcomes.

Providers will perform point-of-care testing, coordinate with physicians and care teams, and deliver care at members’ homes or Skilled Nursing Facilities.

Qualificações

• Master’s Degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority.
• Board certification in Adult, Family, or Gerontology specialties.
• Current BLS/ACLS certification.
• 1 year NP experience preferred; new grads encouraged.

Responsabilidades

• Perform adult/geriatric health assessments and document findings per Matrix guidelines.
• Order and administer point-of-care and lab tests as needed.
• Collaborate with PCP, case managers, social workers, and caregivers.
• Provide patient education and follow-up planning with PCP collaboration.
• Deliver services at the member’s residence, including homes or SNFs.
