Matrix Medical Network provides in-home comprehensive health assessments to older adults and at-risk individuals. Providers review health history, medications, and social factors in members’ homes, delivering insight that may close care gaps.

The CHA service supports clinicians and physicians with follow-up and targeted referrals. Role emphasizes one-on-one visits in members’ residences, with accompanying training, licensure support, and a pathway to full-time opportunities.

Competenze

• Master’s degree and NP licensure in state of practice.
• Current NP licensure with prescriptive authority or ability to obtain.
• Board certification in an adult- or gerontology-focused specialty.
• Current BLS/ACLS or CPR certification.
• 1 year NP experience preferred; new grads welcome.
• Valid state driver’s license and auto insurance coverage.

Mansioni

• Conduct adult/geriatric assessments including medical history and diagnoses.
• Administer point-of-care tests and order lab work as needed.
• Collaborate with physicians, case managers, social workers and caregivers.
• Provide education and follow-up with PCPs and patients.
• Perform services at members’ homes or care facilities.
