Job summary
Matrix Medical Network seeks a licensed Nurse Practitioner to perform comprehensive in-home health assessments (CHA) for adults and geriatric members. You will review medical history, medications, and social factors, and coordinate with physicians and care teams to close care gaps.
The role offers flexibility in scheduling, potential for full-time positions, and competitive compensation with benefits. Providers visit members in their homes or skilled nursing facilities as needed.
Qualifications
- Master’s Degree in nursing or commensurate experience with NP licensure.
- Current NP licensure in state of practice with prescriptive authority or ability to obtain.
- Board certification in Adult/Adult Gerontology/Family/ACNP/EM/acute care.
- Current BLS, ACLS, or CPR certification.
Responsibilities
- Conduct adult/geriatric health assessments including medical history and diagnoses.
- Order and administer point-of-care and lab testing as needed.
- Collaborate with physicians, case managers, social workers and family members.
- Educate patients and provide follow-up with PCPs.
- Provide in-home health assessments at members' residences, including SNFs.
